Request Tax Form Amendment
Please follow these steps to Request a Tax Form Amendment on the Student Portal.
Submit Amendment Form
- Browse to the Student Portal
- Sign in to the Student Portal (KB Article)
- Select My Profile from the list of menu options
- Submit your Social Insurance Number (SIN), if you have not already
- Complete the following form to Request an Amendment:
https://continue.yorku.ca/my-account/request-tax-form-amendment/
Tax Amendment requests will be reviewed and/or processed within 3-5 business days.
